Clinics, practices, labs and pharmacies handle the same short notes many times a day: who saw a form, when results arrived, whether a referral went out. A medical stamp turns those notes into one clear, uniform impression instead of handwriting that varies from person to person. The templates on this page give you a starting layout for provider stamps, practice stamps and status marks, which you can edit online and export as a digital image or as artwork for a stamp manufacturer.

Most medical stamps fall into a few groups:

• Provider identification: the clinician's name, credentials and practice details on letters, forms and paper records.

A stamp does not by itself make a prescription, sick note or medical certificate valid. Rules on what a prescription or certificate must contain, whether a stamp is accepted, and whether a handwritten or electronic signature is required differ by country, state and profession. Follow the rules of your licensing body or health regulator, and treat the stamp as a convenience that sits alongside those requirements, not a replacement for them.

For a provider or practice stamp, typical fields are:

• Practitioner name and credentials, for example "Dr. Jane Smith, MD" or "A. Patel, RN"
• Specialty or department, such as "Family Medicine" or "Pediatrics"
• Practice or clinic name, street address and phone number
• A registration or license number, only where your regulator requires or allows it on documents

For status stamps, keep the wording to a short action and a place to record who and when, such as "Lab Results Received · Date ____ · Initials ____" or "Reviewed by ____ on ____".

Rectangular stamps are the usual choice for provider details and status marks, because three or four straight lines of text read quickly and fit into form boxes. A common provider stamp is about 2 to 2.5 inches wide and under an inch tall (roughly 50 to 65 mm by 20 to 25 mm). Measure the signature or stamp box on the forms you use most and design to that space.

Round stamps work for a practice or department mark, with the clinic name curved around the edge and the city or specialty in the center. Keep the diameter large enough, often 1.5 inches (38 mm) or more, for curved text to stay legible.

For icons, a simple stethoscope or the Rod of Asclepius is common. Avoid the red cross on a white background: it is a protected emblem in most countries and is not for general medical branding.

1. Pick a template with the right shape and number of lines, then set a standard or custom size to match your form boxes.
2. Replace the sample text with your name, credentials and practice details. Use a plain sans-serif font at a size that stays readable when the form is copied, scanned or faxed.
3. On round templates, adjust the curved text (font, size, letter spacing and position) so the clinic name sits evenly around the ring.
4. Choose a single or double border. Dashed, dotted and vintage borders look informal and can break up on scans.
5. Set the ink color. Black or dark blue is the safest choice for documents that will be photocopied. Keep opacity at 100% and the aging effect off so the mark stays crisp.
6. Add a line shape for a signature or date line if you want staff to complete the stamp by hand.
